The following forms can be downloaded to your computer and printed at home. By filling out these forms ahead of time, you will help us ensure that we are able to see all of our patients in a timely manner. If you have any questions when filling out these forms, please do the best you can and our staff will assist you on the day of your visit.

- Insurance billing
Insurance:
We accept most major insurance plans. If you do not see your plan listed here, please contact our office to verfiy whether we accept your insurance. Additionally, insurance plans vary, please contact your insurance company for coverage and referral information.

We are unable to accept the following vision plans:
- VISION SERVICE PLAN (VSP)
- SPECTERA
- UNITED VISION
- EYE MED VISION PLANS
- DAVIS VISION
- REGENCE FEP BLUEVISION
- CIGNA VISION
Some health plans (such as Premera and Regence) contract their vision benefits out to these companies. Please check with your insurance company if you are unsure.
